We are now accepting NHS patients who live outside our catchment area. In the event of a home visit being required patients living outside of the Catchment Area will be visited by a GP from a practice nearer their location.
If you are on medication please book an appointment with the doctor for a medication review when you attend to register. Your medical notes may well take a couple of months to arrive and we cannot prescribe for newly registered patients for whom we do not yet have the notes and have not been reviewed by a doctor.
There are now two ways to register- either by completing the entire process online or by coming in and completing the "in person" registration. Either results in full registration- it is purely personal preference.
Online registrations only
You are now able to fully register online using the following links:
Adult: Online Patient Registration
Children: Online Registration (Children under the age of 16 years)
We encourage anyone who can to register online as this avoids unnecessary commuting and waiting - it is also quicker and can be done at your convenience 24 hours a day.
Please email us a proof of your address. This should be a document such as a utility bill or bank statement. It should be from the last four months and must be in your name. You will also need to email us in an official document with photo showing your identity such as a passport or UK driving licence and the completed patient health questionnaire. The email address to send the required docmentation is firstname.lastname@example.org.
Please allow two working days for us to process your registration application (after submitting your online registration - with the required documentation) prior to booking an appointment. If you need anything more sooner please call the surgery and speak to a member of reception.
In-Person registrations (the rest of the page deals only with in-person registrations- no need to read if you are completing the online registration)
Alternatively, registration can be done in person at reception. Please arrive between the following times to register:
- Monday- 08:30-12:30 , 14:00-17:30
