We are now accepting NHS patients who live outside our catchment area, however we are unable to provide home visits* to those outside our boundaries. Please check the Practice Map for this.
Registration can be done at reception. Please arrive between the following times to register:
- Monday- 08:30-12:30 , 14:00-17:30
- Tuesday- 08:30 - 17:30
- Wednesday- 08:30-11:30, 14:00- 18:30
- Thursday- 09:00 - 18:30
- Friday- 08:30 - 12:30, 14:00 - 17:30
In order to facilitate the registration process, please complete the pre-registation section before you come in.
Please bring in proof of your address. This should be a document such as a utility bill or bank statement. It should be from the last two months and must be in your name. You will also need to bring in an official document with photo showing your identity such as a passport or UK driving licence and the completed patient health questionnaire.

Copies of these forms are also available at reception for those without access to a printer.
If you are registering as a family, you all need to attend, including children.
If you have been registered with an NHS doctor before please ensure you bring in your NHS Number- this can be found on your medical card. If you have lost or misplaced your medical card, you can obtain your NHS number by phoning your previous GP. To register you we will need the details of your previous GP, please ensure you have these when registering.
If you are on medication please book an appointment with the doctor for a medication review when you attend to register. Your medical notes may well take a couple of months to arrive and we cannot prescribe for newly registered patients for whom we do not yet have the notes and have not been reviewed by a doctor.
