Doctor's Letters

 

We are aware that patients are often asked to provide a "doctors letter" to document how an illness impacts on a patient’s life.

For example:

  • Exam performance / mitigating circumstances
  • Housing needs
  • Work needs
  • Supporting Letters - application for benefits such as PIP appeal, blue badge.
  • Fitness to fly

Provision of these types of signed doctor’s letters falls outside of the remit of the NHS and are chargeable with a cost between £40.00 to £80.00, details of which can be found on our Non NHS Services page. These have to be completed outside of NHS clinic hours and can take up to 10 working days to complete.

We accept the following  payment methods:  cash, bank transactions or card payment.
Please note: There will be an additional charge of £2.53 for any card transaction to cover the fees charged by our card provider (Square). For cash, we will require the EXACT payment. We do not keep change on surgery premises.

Please be aware that we do not offer refunds if you are not happy with the completed letter. The payment is to cover the time for the Doctor to complete the letter. They will only be able to state what is factually correct and this will be written in the way the Doctor feels comfortable. Further amendment requests are at the discretion of the doctor and will be chargeable.

Self-Reporting Patient Letter 

In the majority of cases, we are unable to independently verify the impact a condition has on a patient’s life and therefore just end up documenting the patient’s statement. We do not feel that this is a good use of a clinician’s time or a patient’s money. Therefore, we have developed a self-reporting letter template.

This enables you to submit a statement regarding any functional ability or illness that you wish to be included in your medical records and then provided to you on a headed letter from the practice. 

There is a £10 fee to process this type of request which is payable prior to completion. Once you have paid for your letter to be processed, it should be ready to collect after 3 working days. 

The headed letter will have the statement that you have submitted and details of whom you would like it addressed to along with your name, NHS number and date of birth. 
It will not be validated or signed by a clinician. 

Please ensure the statement and addressee details provided by you are complete as any adjustments required after the initial letter has been printed and signed will be subject to a further charge of £5.

Please note that we cannot accept any responsibility for self-reported content. Any requests/letters that are deemed inappropriate will be declined.

As an alternative to this patient statement, or as an addition, you can request a summary printout of your medical notes which will have an unfiltered list of medication and all your ongoing and recent problems which is free.

We do not consider letters to be clinically urgent. If you require a letter urgently (ie within 24 hours) we charge an additional administration fee of £30.00.

Please note that clinician’s appointments should not be booked for the purposes of writing letters or reports, they should only be used for providing Clinical NHS care.
All letters, forms or reports should be collected in person. If you wish for the letter to be posted to you, please provide a stamp addressed envelope.

Fitness to Fly in Pregnancy

To be able to confirm someone is fit to fly in pregnancy we require access to maternity notes. We no longer have this since these are now held in a digital form not accessible to us.

If you do have handheld maternity notes (ie not digital) then you can drop them off at the surgery for us to view, otherwise you need to ask your midwife to confirm in writing

  1. what stage of pregnancy you are, ie when your estimated due date is (EDD)
  2. whether there have been any issues in your pregnancy 
  3. if in their opinion you are fit to fly.

So in summary, if we cannot see a written statement from your midwife, we will not have information to base a fit to fly letter on.

Please ask your midwife to issue this to you. Please also check with your airline exactly what information is needed.
